Can you help identify this model please?

Hi, I'm hoping the VC community might offer me some advice. 


I've inherited this watch and I've been trying to work out when it was made and what model it is. 


At the risk of sounding naive, what is the significance  of the 2 smaller hands? I can't see many others with this feature for sale. Does it affect the value? I'm told the movement is original and works correctly. 

Any advice /resources would be much appreciated.

elapsed minutes on the upper totalizer wink.  The large central hand shows elapsed seconds on this lovely 2-register pocket chronograph.
